Dr. Andrea Cercek, one of the lead researchers, said: “It’s absolutely incredible … We’ve certainly never seen this before.” All 12 patients experienced complete rectal cancer remission after receiving dostarlimab, a monoclonal antibody. This particular monoclonal antibody achieved 100% cancer remission without chemotherapy, radiation, surgery, or the toxic side effects that come with such therapies. The authors wrote: “All 12 patients (100%; 95% confidence interval) had a clinical complete response, with no evidence of tumor on magnetic resonance imaging … and no cases of progression or recurrence had been reported during follow-up (range, 6 to 25 months after treatment). No adverse events of grade 3 or higher have been reported.” How does this monoclonal antibody actually work? Dostarlimab is a monoclonal antibody, a type of drug designed to target specific proteins in the body. It works by boosting your immune system’s ability to fight cancer, particularly in cases like the rectal cancer patients with mismatch repair-deficient (dMMR) tumors. Here’s how it pulls that off: Normally, your immune system uses T-cells—think of them as the body’s attack dogs—to spot and destroy abnormal cells, like cancer. But cancer’s sneaky. It can hijack a protein called PD-1 (Programmed Death-1) on T-cells. When PD-1 binds to another protein, PD-L1, which is often overexpressed on cancer cells, it sends a signal that basically says, “Nothing to see here, move along.” This shuts down the T-cell attack, letting the cancer hide in plain sight. Dostarlimab steps in as a checkpoint inhibitor. It’s engineered to latch onto PD-1 and block that interaction with PD-L1. By doing so, it takes the brakes off the immune system. The T-cells stay active, recognize the cancer cells as threats, and go to work destroying them. In dMMR tumors, this is especially effective because these cancers have a ton of mutations—hundreds or thousands more than typical tumors.
